Consent to Photograph
We love sharing our patients’ stories, but want to protect your privacy. When taking photos in the building, we are careful to keep patients unrecognizable unless they give permission to be filmed. We ask every patient or visitor who agrees to be photographed or filmed to complete our consent form.
Diabetic Blood Sugar Log
Designed for patients working alongside our Certified Diabetes Care and Education Specialists (CDCES) to log metabolic baselines.
